Payment Policies
Each insurance company and plan is different. It is best to check-in with your insurance company prior to your visit to see if they require a referral and if our practice is in-network - if we are out-of-network with your insurance we will still be happy to see you, but you may be responsible for a larger amount of the visit cost. We do require patients to pay their co-pay at the time of visit. If your insurance plan is one with a decuctible that has not been met, we will ask that $75 towards the deductible is given on the first visit, and $50 towards the deductible for all other subsequent visits. If you do not have insurance, you will be given the opportunity to do a cash-pay. This is given with a slight discount and the entire amout is to be collected before you leave our office.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you; this includes bringing your insurance cards, photo ID, and medication list.
